Graue Rückenmarksflüssigkeit verstehen: Ursachen und Implikationen

Graue (xanthochromische) Hirn-Rückenmarksflüssigkeit (CSF) deutet typischerweise auf eine Subarachnoidalblutung (SAH) hin – rote Blutkörperchen-Lyse setzt Hämoglobin-Abbauprodukte (Bilirubin, Oxyhämoglobin) frei. Ursachen:

  • Aneurysmale SAH: Geplatzte intrakranielle Aneurysma (häufigste nicht-traumatische).
  • Traumatische SAH: Kopfverletzung.
  • Arteriovenöse Malformation (AVM) Ruptur.
  • Selten: Pigmentierte Meningitis (Melanom), schwere Hyperbilirubinämie.

Kritische Maßnahmen

  1. Sofortige neurochirurgische/neurologische Konsultation.
  2. CT Kopf (ohne Kontrast) – erkennt akutes Blut >95 % innerhalb von 6 h.
  3. CT-Angiographie / Digital-Subtraktions-Angiographie für Aneurysma/AVM.
  4. Lumbalpunktion, wenn CT negativ, aber klinischer Verdacht hoch (Bilirubin-Spektrophotometrie bestätigt SAH >12 h post ictus).

Zeit ist Gehirn; aneurysmale SAH hat hohe Mortalität/Morbidität ohne rasche Intervention.
