Neuropathie (periphere Neuropathie) ist Schädigung peripherer Nerven, die sensorische, motorische oder autonome Dysfunktion verursacht. Sie kann eine Mononeuropathie (ein Nerv), Multiplex oder Polyneuropathie (symmetrisch, längenabhängig) sein.
Wichtigste Ätiologien
- Diabetes mellitus: Hauptursache. Metabolisch (Polfunktionsweg, AGEs, PKC, oxidativer Stress) + vaskuläre Verletzung. Distal symmetrisch sensomotorisch ± autonom.
- Alkohol: Toxisch + ernährungsbedingt (Thiamin). Schmerzhafte sensorische > motorische.
- Ernährungsbedingt: B12-Mangel (subakute kombinierte Degeneration), B1, B6 (Übermaß), E, Kupfer.
- Autoimmun/Entzündlich: GBS, CIDP, vaskulitisch (PAN, Kryoglobulinämie), paraneoplastisch, Sjögren, Lupus.
- Hereditär: Charcot-Marie-Tooth (CMT1A Dup 17p12), HNPP, familiäre Amyloidneuropathie.
- Toxisch: Chemotherapie (Platin, Taxane, Vincristin, Bortezomib), Schwermetalle (Blei, Arsen, Quecksilber), Medikamente (Metronidazol, Isoniazid, Linezolid).
- Kompressiv/Traumatisch: Karpaltunnelsyndrom, Radikulopathie, Thoracic-Outlet-Syndrom.
- Infektiös: HIV, Borreliose, Lepra, Diphtherie, CMV.
- Systemisch: Amyloidose, Nieren-/Leberversagen, Hypothyreose, kritisch-kranke Neuropathie.
Symptome
- Sensorisch: Taubheit, Kribbeln, Brennen, Schmerz (neuropathisch), Allodynie, Verlust von Vibration/Position.
- Motorisch: Schwäche, Atrophie, Faszikulationen, Krämpfe, Fußheberschwäche.
- Autonom: Orthostatische Hypotonie, Gastroparese, neurogene Blase, erektile Dysfunktion, Anhidrose.
Diagnose
- Anamnese, neurologische Untersuchung (Strumpf-Handschuh-Verteilung, Reflexe, Propriozeption).
- NLG/EMG (demyelinisierend vs. axonal, Verteilung).
- Labor: Glukose/HbA1c, B12, Folsäure, TSH, SPEP/Immunfixierung, ANA, BSG, Schwermetalle, Gentest-Panel.
- Hautbiopsie (kleine Fasern), autonomes Testing, Nervenbiopsie (selten).
Behandlung
- Ätiologie-spezifisch: Glukosekontrolle, B12-Ersatz, Immunsuppression (CIDP), Plasmapherese (GBS), Toxinentfernung.
- Symptomatische neuropathische Schmerzen: Gabapentinoide, SNRI (Duloxetin), TCAs, topisches Lidocain/Capsaicin.
- Physiotherapie, Orthesen, Sturzprophylaxe.
- Fußpflege (diabetisch): tägliche Inspektion, geeignetes Schuhwerk, Podologie.
Frühe Diagnose verhindert irreversiblen Axonverlust. Multidisziplinäre Versorgung optimiert Funktion.
